Pay: $27-$40 per hour, DOE
GENERAL SUMMARY
Under the general direction of the COHE Manager, the Health Services Coordinator (HSC) plays a critical role within the COHE by working directly with injured workers, employers, health care providers, and other program participants to navigate the WA worker's compensation system. This work includes functioning as a liaison on behalf of the attending provider, and facilitating communication between the injured worker, employer, and claim manager ensuring questions and issues are addressed.
ESSENTIAL FUNCTIONS
The job duties listed are essential functions of the position. However, other duties may be assigned and may also be considered essential functions of the position.
The caregiver must be sufficiently fluent in the English language to satisfactorily perform the essential functions of the position.
The degree of fluency required will vary depending upon the nature of the position.

For direct patient care roles: Performs and maintains currency of essential competencies as required by specific area of hire and populations served.
Key Responsibilities
• Advocates for all parties and serves as a liaison on behalf of the provider to ensure the injured patients' questions and issues are addressed.
• Assists providers, workers, and employers with all aspects of L&I standard work categories to improve clinical outcomes.
• Ensures all required forms and documents are completed by the responsible party in accordance with established best practice procedures. Checks to ensure providers have completed all required fields with sufficient information on the Report of Accident (ROA) and Activity Prescription Form (APF)to ensure claims are unimpeded.
• Tracks claim requests received and responds timely to questions from doctors, employers, workers, or L&I.
• Completes outreach and education efforts to potential new clinics in assigned territory.
• Delivers professional presentations for appropriate groups promoting participation and/or support for the COHE program. This includes Healthcare facilities, along with business and labor groups.
• Collects and organizes data that monitors the effectiveness of the COHE Program. Distributes reports and explains results for continue improved efficiencies.
• Demonstrates commitment to continuous quality improvement in all job responsibilities. Participates in performance improvement activities and contributes to program development to facilitate accomplishment of departmental goals.
• Maintains a list of participating employers and labor unions for the COHE.
• Is a positive representative of our client at all times. Is pleasant, respectful, and courteous in all interactions with all stakeholders and peers in representing the COHE program. Demonstrates behaviors that reflect our client's mission and values.
• Data management and tracking. Completing care coordination or similar documentation. Analyzing and communicating provider, care coordinator, and best practice reporting data.
• Tracking specific claims to ensure early return-to-work services, care coordination, improved clinical outcomes of injured workers, identifying barriers to return-to-work and developing strategies to address them.
• Provides outreach and trains providers and clinic staff on the COHE program deliverables and best practice procedures. Training may require minimal to moderate daytime travel with an occasional overnight stay.
EDUCATION
Required: Bachelor's Degree
Preferred: Master's Degree
Education/Experience Equivalencies:
If no bachelor's degree, 7 years' experience is required in direct patient care, return-to-work coordination, occupational health care, or equivalent experience (direct patient care includes nurse, medical assistant, nurse navigators, and existing care coordinators, but does not include referral coordinators, schedulers, or clinical administrative staff).
EXPERIENCE
Required - 3 Years Minimum
• Experience in direct patient care, return-to-work coordination, occupational health care, or equivalent experience.
• Experience in data management and tracking.
• Experience completing care coordination or similar documentation.
• Experience analyzing and communicating provider, care coordinator, and best practice reporting data.
Preferred - 1 Year Minimum
• Experience as a Vocational Counselor, Nurse Case Manager, or accredited Case Manager working with injured workers in an industrial insurance system or health care setting.
• Equivalent experience in a health care setting, such as a Physical Therapist or Occupational Therapist.
